Booking Form and Contract for Photography Workshops

Thank you so much for choosing to book a Photography Workshop with me. Please fill in all of the details requested and then read through the below Terms & Conditions carefully. Once you’re happy please submit this from. I will then get a notification and email you back a copy of the form for your records along with an invoice for the fee.
  • To attend one of these workshops you will need to bring a digital camera capable of manual control. Please list the make and model of the camera you intend to bring. If you're unsure whether the camera you have is suitable please contact me. Please bring the camera manual with you if you have it.
  • If you have any additional comments on your current skill level or experience please list them here.

  • Details of what will be contained in the course will be made available in due course and will be aimed at making you able to use your camera in full manual mode by the end of the day. However, if there is anything you particularly want to learn please give details here and I will try to include it in the course.
  • Tea, coffee, soft drinks, biscuits and lunch will be provided as part of the course. Please use this box to let me know if you have any dietary requirements.
    1. The acceptance of the booking outlined above, once completed, is based on agreement by the client of the terms and conditions stated herein.
    2. The full fee is required to be settled within 7 days of the placement of the booking and signing of this agreement.
    3. Cancellation less than 4 weeks before the date of the event will require payment in full. All cancellations must be in writing. The cancellation conditions above will apply for postponed events if the photographer is not able to re-schedule for the new date and time. The fee for postponed events may be subject to change.
    4. In the unlikely event that the photographer is unable to hold the workshop due to unforeseen circumstances, the photographer reserves the right to reschedule the workshop at a mutually agreeable time.
    5. As part of the workshop you will be provided a Course Manual in PDF format. The Course Manual is for your personal use only. You are expressly prohibited from reproducing, copying, editing, transmitting, uploading or incorporating in any other materials, any of the Course Manual.
    6. It is understood that the client may be photographed by Matthew Long Photography whilst attending the workshop. The client gives permission to Matthew Long photography to use any image of our event as required and for general promotional use for Matthew Long Photography.
    7. Matthew Long Photography is entitled to use all images produced for exhibitions, editorials, digital and print marketing, publications educational DVDs, websites and/or display purposes.
